All cremations require the next-of-kin (NOK) to authorize the release of your loved one to our care. This preliminary form may be downloaded from this website. The Authorization for Cremation and Disposition Form are available to you through U.S. Mail or facsimile.
In addition, the vital statistics information needed for us to file the Certificate of Death is quite specific and, at the very least, requires a meeting with our Director, Mr. Geronimo Mena, Jr. If a meeting is not obtainable for a few days, then this information must be taken by phone.
